Our office services adult and geriatric patients in Philadelphia, Bucks, Montgomery and Delaware counties.
Several Immediate PRN positions available to RNs with Home Care Experience.

Registered Nurse (RN) Duties:
Follow patient's plan of care in accordance to doctor's orders Travel to patient homes as assigned and manage own schedule Home care visit duties may include:
medication administration, wound care, catheter changes, disease management and education Monitor, observe, evaluate and manage patients' conditions, report to physician and properly document Perform patient assessments, evaluations and Start of Care Admissions Case manage, follow up and coordinate with care team, family and physician Qualifications for a Registered Nurse (RN):
A current license as a Registered Nurse in good standing and able to practice in Pennsylvania.
A minimum of one year of recent, verifiable RN clinical (medical/surgical) experience.
A Minimum one year prior home health care experience providing home visits Graduation from an accredited and approved nursing program Strong interpersonal skills, excellent work ethic and communication skills Experience with electronic medical records (EMR) preferred.
Knowledge of Axxess software is a plus.
